01Mechanism of Action
Parameter
FOXO4-DRI
Semaglutide
Primary target
FOXO4-p53 protein complex in senescent cellsBourgeois 2025Tripathi 2021
GLP-1 receptor (GLP-1R)WEGOVY (semaglutide) injection 2021
Pathway
FOXO4-DRI binds disordered p53 transactivation domain → displaces FOXO4 → nuclear p53 exclusion → p53-mediated apoptosis in senescent cells
GLP-1R agonism → ↑glucose-dependent insulin secretion, ↓glucagon, ↓gastric emptying, ↓appetite via hypothalamic centresWilding 2021
Downstream effect
Selective apoptosis of senescent cells; clearance restores tissue homeostasisTripathi 2021Alameen 2026
Improved glycemic control, reduced caloric intake, body-weight reduction, cardiovascular risk reductionWilding 2021
Feedback intact?
—
Glucose-dependent insulin release preserves physiological feedback
Origin
D-retro-inverso modification — inverted amino acid sequence, D-amino acids for protease resistance
Modified GLP-1(7-37) with two amino-acid substitutions and C-18 fatty-acid acylation for albumin binding and 168-h half-lifeWEGOVY (semaglutide) injection 2021

Absolute Contraindications
FOXO4-DRI
- ·Pulmonary hypertension or vascular disease (preclinical evidence of harm)Born 2023
- ·Pregnancy / lactation (no safety data)
Semaglutide
- ·Personal or family history of medullary thyroid carcinoma
- ·Multiple endocrine neoplasia syndrome type 2
- ·Pregnancy / breastfeeding
- ·Hypersensitivity to semaglutide
Relative Contraindications
FOXO4-DRI
- ·Active malignancy (senescence as tumour suppressor mechanism)
- ·Wound healing / tissue repair (senescent cells involved in fibrosis resolution)
Semaglutide

05Administration Protocol
Parameter
FOXO4-DRI
Semaglutide
1. Pre-clinical route
Subcutaneous injection used in rodent models. No human administration protocol exists.
Commercial: pre-filled pen, no reconstitution. Research vial: per-label or bacteriostatic water.
2. Reconstitution (animal)
Typically reconstituted in sterile saline or PBS for animal experiments. Stability data limited.
SQ — abdomen, thigh, or upper arm. Rotate sites weekly to avoid lipohypertrophy.
3. Dosing schedule
Variable — single bolus or intermittent dosing over weeks. No standardised human protocol.
Once weekly, same day. Day can be changed if ≥2 days separate doses.
4. Clinical development status
No registered human trials. Commercialisation by Cleara Biotech (Netherlands) in development phase.
Pen: refrigerate 2–8 °C unopened; room temp ≤30 °C up to 56 days after first use.
5. Safety monitoring (proposed)
Would require cardiovascular assessment, pulmonary function, immune panel, tumour surveillance if human trials proceed.
Pen-supplied 31–34G needle. Research vial: 27–31G insulin syringe.
06Stack Synergy
FOXO4-DRI
— no documented stacks
Semaglutide
+ Tirzepatide
WeakCombining two GLP-1 RA-class drugs is not clinically validated and risks additive GI toxicity. Tirzepatide's GIP component already provides complementary mechanism vs pure GLP-1; stacking with semaglutide adds receptor saturation but no synergy. NOT recommended.
- Note
- Stack not recommended — choose one GLP-1 RA
- Primary benefit
- (none — additive toxicity, no synergy)
