01Mechanism of Action

Parameter
MK-677
Tirzepatide
Primary target
Ghrelin receptor (GHS-R1a)Murphy 1998
GIP receptor (GIPR) + GLP-1 receptor (GLP-1R)Frias 2018
Pathway
GHS-R1a → Gαq → Ca²⁺ → sustained GH pulses across 24 hrNass 2008
Dual GIPR/GLP-1R agonism → ↑insulin (glucose-dependent), ↓glucagon, ↓gastric emptying, ↓appetite, ↑energy expenditure (via GIP component)Jastreboff 2022Frias 2018
Downstream effect
Sustained GH + IGF-1 elevation; appetite stimulation; lean mass preservationNass 2008
Profound glycemic improvement and weight reduction; cardiometabolic benefitsJastreboff 2022
Feedback intact?
Pulsatile pattern preserved despite long half-lifeMurphy 1998
Glucose-dependent insulin release preserves physiological feedback
Origin
Non-peptide spiroindane-piperidine small molecule designed at MerckMurphy 1998
39-AA peptide with C-20 fatty-acid acylation. Single molecule with balanced GIP + GLP-1 affinityFrias 2018

02Dosage Protocols

Parameter
MK-677
Tirzepatide
Standard dose
10–25 mg / day oralNass 2008
25 mg used in Nass 2008 elderly trial; 10–15 mg common community dose.
Frequency
Once daily, oral
Lower / starter dose
5 mg / day
Evidence basis
Phase 2 trials (Nass 2008, Murphy 1998)Nass 2008Murphy 1998
FDA-approved · Phase 3 RCTs (SURMOUNT, SURPASS)Jastreboff 2022ZEPBOUND (tirzepatide) injecti 2023
Duration
8–16 weeks per cycle (off-cycle to reset receptor sensitivity)
Indefinite for chronic indication
Reconstitution
Oral, no reconstitution
Pre-filled commercial pen. Research vial: bacteriostatic water per label.
Timing
Pre-sleep preferred for natural GH pulse alignment
Once weekly, any time of day
Half-life
~24 hrNass 2008
Once-daily dosing covers 24 hours.
Standard dose (T2D)
Standard dose (weight)
Titration schedule
2.5 mg → +2.5 mg every 4 weeks → 15 mg max
Slower titration mitigates GI side effects.
